The following trademark service mark application and renewal application are the required forms for applying for registration and renewal, respectively.

The trademark/service mark application must be accompanied by three specimens showing the mark as actually used.   The renewal application must be accompanied by one specimen showing current and continued use of the registered mark.

The assignment form may be used to change ownership of a registered mark.  While the standardized form is recommended it is not mandated by statute; we may accept other legally binding documents when recording an assignment.
