For most companies, four items are necessary to complete the reinstatement process. All four steps must be completed in order to reinstate your corporate entity.
1) A letter of good standing of good standing from the Kentucky Department of Revenue. You must obtain a letter of good standing from the Corporation Income and License Tax Branch of the Kentucky Department of Revenue. This letter will confirm that all taxes have been paid and that your company is eligible for reinstatement. You must provide a copy of this letter with your reinstatement information to our office.

2) A letter of good standing of good standing from the Kentucky Division of Unemployment Insurance. Effective July 1, 2008, pursuant to KRS 271B.14-220(1)(e), a letter of good standing from the Kentucky Division of Unemployment Insurance. This letter will confirm that all employer contributions, interest, penalties, and service capacity upgrade fund assessments have been paid and that your company is eligible for reinstatement. You must provide a copy of this letter with your reinstatement information to our office.

3) The Application for Reinstatement. Click on the following link to access the Online Business Database. To access your reinstatement application, please enter your organization id or company name in the space provided on the business database search. Click on the reinstatement packet button at the top of the page and your personalized reinstatement application will be created for you. Depending on your entity type, the materials necessary to complete the reinstatement process will be listed as links at the top of the page. Simply click on the links to the individual forms.
Quick Tip: For most companies, after printing your reinstatement application, be sure to return to the previous page to view this list and to print the reinstatement annual report.
4) The Reinstatement Annual Report. Your reinstatement annual report is created the same way as the application for reinstatement, by using the Online Business Database. If your company needs to file a reinstatement annual report, please enter your organization id or company name in the space provided on the business database search. Click on the reinstatement annual report button at the top of the page and your personalized annual report will be created for you.
Also, if you are making any changes to the registered agent, the registered office or changing the principal office address, you will need to complete one or both of the following forms. The forms are available on our
